- Any Doctors letters, reports, notes, instructions or consent form
- An up-to-date list of your current medication signed by your GP
- All medication currently being taken, in the original packaging with your name, the drug name and administration instructions on the label. Include inhalers if you use them
- All medication repeats and authority scripts
- All relevant x-rays and scans
- If you have sleep apnoea and use a CPAP machine please bring this machine to hospital
Cards
- Family prescription record card
- Health Care Card, Pharmaceutical Safety Net Card
- Health Fund card or book, DVA Card for veterans
- Medicare Card and Pensioner concession card
- Third Party/Accident or Workers Compensation Claims details
Personal belongings
- Nightwear, dressing gown, well fitted non-slip slippers
- Pillow (if you prefer to use your own)
- Mobile phone or other digital device (can be used to communicate with friends or family via our WIFI network, or access newspapers online)
- Reading material (and spectacles if required)
- Toiletries
- Walking aid (if used)
